Why are we asking for this information?
The MAA requires us to report on the meeting including statistics about the number of participants and speakers in various categories. Asking this explicitly saves much work over trying to deduce this from first names, email addresses etc.
The information will only be used for statistics and has no impact on the selection or scheduling of talks.
